HABILITATION
Habilitation is a service that is funded by the Arizona
Department of Economic Security's Division of
Developmental Disabilities (DDD). It is designed to help
individuals with disabilities to learn new skills for
everyday living. Skills are learned by working on
objectives that are designed to help them with their
goals.
Habilitation is a broad service that can have a wide variety
of goals. Goals can be things like:
At Community Care Solutions, we take a very person-
centered approach with Habilitation. We write an
individualized Habilitation Plan for each and every client
we serve. Each plan is designed with different goals in
mind according to what the client would like to achieve.
In order to reach that goal, we help write objectives to
work on that will help the individual to reach the desired
outcome.
We review the goals and objectives with the individuals
being served, family members, and the providers as often
as necessary, but no later than every 90 days in order to
ensure that the goals are being reached and to modify the
plans if necessary. Our providers document progress
each month so that we can send this report to the DDD
Support Coordinator and help to monitor progress.

- learning how to ride the bus to work
- learning new recreational activities
- learning how to cook
- practicing social skills
